"content": "(i) within twenty days from the date of birth or still birth ; and\n(ii) within three days from the date of death ;\n(i) \"the information in respect of births and deaths in a house referred to in Clause (a) of sub-section (1) of Section 8 shall be given to the village level worker or the teacher, as the case may be, in whose jurisdiction the house falls ;\n(ii) the village level worker or the teacher, as the case may be, shall receive information in Form Nos. 2, 3 and 4 for registration of a birth, still birth and death respectively if given in writing, and if given orally shall record the several particulars in relevant form and obtain informant's signature and if he or she cannot write, his or her thumb mark in the relevant form ;\n(iii) the village level worker or the teacher, as the case may be, shall give or cause to be given such forms to the Registrar within 3 days of the receipt of report.",
